| To enrol your child, please print the downloadable application form, fill it out and return it to us together with a deposit of £25.00 to secure your child's place. Please make cheques payable to: Theatre4Kids |

Once your application has been processed you will receive a Theatre4Kids welcome pack confirming your child's place, details of how to find us at Danesfield Manor School and what your child will need to bring along for the first lesson. Your £25.00 deposit will cover a trial period of 2 weeks and is non-refundable. If in the event the school is full and you do not wish your child to go on the waiting list Theatre4Kids will happily refund your deposit. |

| The Personal
Data included in the application form will be "processed"
(as such terms are defined in the Data Protection Act 1998) by Theatre4Kids
for the purposes of administration, research, the provision of teaching
services, the organisation of events and for the administration of Theatre4Kids'
statutory obligations under legislation relating to children. Theatre4Kids
will not disclose this information to third parties for marketing purposes.
Theatre4Kids itself may use this information to provide marketing information.
